Die erste Impfung jetzt. Dann die zweite Impfung in drei Monaten. Und dann mindestens ein halbes Jahr warten bis man vielleicht eine Impfung mit einem richtigen Impfstoff bekommt. Bis dahin können die Vakzin-restistenten Mutanten wüten. Neun Monate lang ist die Bevölkerung zum Abtauchen verdammt. Neun lange Monate.

Der Impfstoff AstraZeneca kann in den bayerischen Arztpraxen auch an
Menschen unter 60 Jahren unabhängig von der Priorisierung verimpft
werden. Bayerns Gesundheits- und Pflegeminister Klaus Holetschek sagte
am Mittwoch in München: „Die Priorisierung bei AstraZeneca ist ab
sofort aufgehoben, der Impfstoff kann in den Arztpraxen auch Personen
unter 60 Jahren angeboten werden. Seit dem 19. April wird AstraZeneca
für Erstimpfungen in Bayern nur in Haus- und Facharztpraxen verimpft.
Die Ärzte kennen ihre Patienten gut, und wissen, wem sie aus dem Kreis
der unter 60-Jährigen unter Berücksichtigung der Vorgaben der
Ständigen Impfkommission ein Impfangebot mit diesem Wirkstoff machen
können und beraten hierzu ausführlich. Dieses besondere
Vertrauensverhältnis wollen wir nutzen, denn jede Dosis Impfstoff muss
möglichst rasch verimpft werden. So schaffen wir für noch mehr
Menschen einen möglichst sofortigen und unkomplizierten Zugang zu einer
Schutzimpfung. Jede Impfung zählt!"

Der Minister fügte hinzu: „Mich freut zudem: Im überwiegenden Teil
der Bayerischen Impfzentren erhalten bereits regelmäßig Menschen aus
der Priorisierungsgruppe 3 ein Impfangebot, d.h. vor allem die 60 bis
69-Jährigen. Das ist ein großer Erfolg! Es zeigt, wie gut die
Bayerische Impfstrategie aufgeht und dass die über 70-Jährigen bereits
weitgehend geimpft sind oder zumindest einen Impftermin erhalten haben.
Jetzt können wir diesen weiteren Schritt zu einem breiteren Impfangebot
machen."

Zur Priorisierungsgruppe 3 gehören nicht nur die 60- bis 69-Jährigen,
sondern etwa auch Lehrkräfte an Gymnasien, Real- und Mittelschulen,
Beschäftigte im Lebensmitteleinzelhandel, Personen, die in besonders
relevanter Position in Einrichtungen der kritischen Infrastruktur tätig
sind wie im Transport- und Verkehrswesen und der Telekommunikation,
sowie Menschen mit Erkrankungen wie Rheuma, Diabetes mellitus ohne
Komplikationen oder Asthma und viele weitere.

Der Minister betonte: „Selbstverständlich werden auch weiterhin in
allen Impfzentren Personen der ersten und zweiten Priorisierungsgruppen
geimpft, wenn bisher nicht Geimpfte in diesen Priorisierungsgruppen sich
nun auch für eine Impfung entscheiden und sich in BayIMCO registrieren,
oder wenn sie die ihnen angebotenen Termine annehmen."

FDA and CDC Lift Recommended Pause on Johnson & Johnson (Janssen) COVID-19 Vaccine Use Following Thorough Safety Review

Agencies Underscore Confidence in Vaccine’s Safety and Effectiveness Following Data Assessment; Available Data Suggest Potential Blood Clots Are Very Rare Events

Following a thorough safety review, including two meetings of the CDC’s Advisory Committee on Immunization Practices, the U.S. Food and Drug Administration and the U.S. Centers for Disease Control and Prevention have determined that the recommended pause regarding the use of the Johnson & Johnson (Janssen) COVID-19 Vaccine in the U.S. should be lifted and use of the vaccine should resume.

The pause was recommended after reports of six cases of a rare and severe type of blood clot in individuals following administration of the Janssen COVID-19 Vaccine. During the pause, medical and scientific teams at the FDA and CDC examined available data to assess the risk of thrombosis involving the cerebral venous sinuses, or CVST (large blood vessels in the brain), and other sites in the body (including but not limited to the large blood vessels of the abdomen and the veins of the legs) along with thrombocytopenia, or low blood platelet counts. The teams at FDA and CDC also conducted extensive outreach to providers and clinicians to ensure they were made aware of the potential for these adverse events and could properly manage and recognize these events due to the unique treatment required for these blood clots and low platelets, also known as thrombosis-thrombocytopenia syndrome (TTS).

The two agencies have determined the following:

Use of the Janssen COVID-19 Vaccine should be resumed in the United States.
The FDA and CDC have confidence that this vaccine is safe and effective in preventing COVID-19.
The FDA has determined that the available data show that the vaccine’s known and potential benefits outweigh its known and potential risks in individuals 18 years of age and older.
At this time, the available data suggest that the chance of TTS occurring is very low, but the FDA and CDC will remain vigilant in continuing to investigate this risk.
Health care providers administering the vaccine and vaccine recipients or caregivers should review the Janssen COVID-19 Vaccine Fact Sheet for Healthcare Providers Administering Vaccine (Vaccination Providers) and Fact Sheet for Recipients and Caregivers, which have been revised to include information about the risk of this syndrome, which has occurred in a very small number of people who have received the Janssen COVID-19 Vaccine.
CDC’s independent Advisory Committee on Immunization Practices met today to discuss the latest data on TTS, hearing from the vaccine manufacturer Janssen and the COVID-19 Vaccine Safety Technical (VaST) Subgroup, as well as a risk benefit analysis. ACIP is committed to be vigilant and responsive to additional information that could impact the risk benefit analysis of any of these vaccines. Vaccine safety monitoring will continue and any new information about TTS will be brought to ACIP as needed.

“Safety is our top priority. This pause was an example of our extensive safety monitoring working as they were designed to work—identifying even these small number of cases. We’ve lifted the pause based on the FDA and CDC’s review of all available data and in consultation with medical experts and based on recommendations from the CDC’s Advisory Committee on Immunization Practices. We have concluded that the known and potential benefits of the Janssen COVID-19 Vaccine outweigh its known and potential risks in individuals 18 years of age and older. We are confident that this vaccine continues to meet our standards for safety, effectiveness and quality. We recommend people with questions about which vaccine is right for them have those discussions with their health care provider,” said Janet Woodcock, M.D., Acting FDA Commissioner.

“Above all else, health and safety are at the forefront of our decisions,” said CDC Director Dr. Rochelle P. Walensky. “Our vaccine safety systems are working. We identified exceptionally rare events – out of millions of doses of the Janssen COVID-19 administered – and we paused to examine them more carefully. As we always do, we will continue to watch all signals closely as more Americans are vaccinated. I continue to be encouraged by the growing body of real-world evidence that the authorized COVID-19 vaccines are safe and effective, and they protect people from disease, hospitalization, and death. I urge anyone with questions about the COVID-19 vaccines to speak with their healthcare provider or local public health department.”

Assessment of Available Data
Medical and scientific teams at the FDA and CDC reviewed several sources of information and data related to the Janssen COVID-19 Vaccine to reach today’s decision.

Specifically, the agencies assessed reports submitted to the Vaccine Adverse Event Reporting System (VAERS), reviewed the medical literature and considered the information from global regulatory partners about thrombosis with thrombocytopenia that have been reported following use of a similar, yet not identical, COVID-19 vaccine using a virus from the adenovirus family that has been modified to contain the gene for making a protein from SARS-CoV-2.

Update on Adverse Events
On April 13, the FDA and CDC announced that, out of more than 6.8 million doses administered, six reports of a rare and severe type of blood clot combined with low blood platelet levels occurring in people after receiving the Janssen COVID-19 Vaccine had been reported to VAERS. In these cases, a type of blood clot called cerebral venous sinus thrombosis (CVST) was seen in combination with low levels of blood platelets (thrombocytopenia).

Today, the agencies can confirm that a total of 15 cases of TTS have been reported to VAERS, including the original six reported cases. All of these cases occurred in women between the ages of 18 and 59, with a median age of 37 years. Reports indicated symptom onset between 6 and 15 days after vaccination.

Monitoring for Safety Will Continue
The surveillance systems that are in place to monitor the safety of COVID-19 vaccines authorized for emergency use are working, as demonstrated by both agencies’ quick work to identify and investigate these rare, but serious adverse events. The FDA and CDC will continue with these efforts to closely monitor the safety of these vaccines.

Reports of adverse events following vaccination can be made to the Vaccine Adverse Event Reporting System.

Mit der "Bundes-Notbremse" ist das Infektionsschutzgesetz in Deutschland geändert worden, um ab einer Inzidenz von 100 landesweit einheitliche Regelungen für den Corona-Lockdown zu schaffen.

Am Donnerstag stimmte - wie der Bundestag bereits am Mittwoch - auch der Bundesrat einer bundesweit einheitlichen Regelung für die sogenannte "Bundes-Notbremse" zu. Anschließend unterzeichnete Bundespräsident Frank-Walter Steinmeier (SPD) das Gesetz und es wurde im Bundesgesetzblatt veröffentlicht - damit ist das neue Infektionsschutzgesetz in Kraft getreten. Die darin enthaltende "Bundes-Notbremse" greift, wenn die Sieben-Tage-Inzidenz in einem Landkreis oder einer kreisfreien Stadt drei Tage hintereinander über den Wert von 100 steigt. Das bedeutet: Der Bund hat per Gesetz Maßnahmen festgelegt - Kommunen müssen diese ohne Wenn und Aber umsetzen. Landkreise, die nicht entsprechend reagieren, handeln dann anders als bislang gesetzeswidrig. Strengere Regeln in den Bundesländern sind weiter möglich.

Zu den Einschränkungen, die ab einer Inzidenz von 100 in betroffenen Regionen in Kraft treten sollen, zählen folgende Punkte - alle erst einmal befristet bis zum 30. Juni:

Strikte Kontaktbeschränkungen: Angehörige eines Haushalts dürfen sich nur noch mit einer weiteren Person treffen.
Nächtliche Ausgangssperre: Anders als zunächst geplant, soll nun nicht schon ab 21 Uhr, sondern erst ab 22 Uhr bis 5 Uhr in den betroffenen Kreisen und kreisfreien Städten eine Ausgangssperre greifen. Draußen aufhalten dürfen sich dann nur Personen, die "begründete Ausnahmen" geltend machen können - etwa zwingende berufliche Gründe oder Notfälle. Joggen und Spaziergänge sollen bis Mitternacht erlaubt sein.
Geschäfte müssen schließen - bis auf die Geschäfte des täglichen Bedarfs wie Supermärkte, Drogerien oder Apotheken. Im Einzelhandel soll unabhängig von der Inzidenz das Abholen bestellter Waren ("Click & Collect") sowie bei einer Inzidenz bis 150 das Einkaufen mit Test und Terminbuchung ("Click & Meet") weiterhin möglich sein. Freizeit- und Kultureinrichtungen sowie die Gastronomie dürfen nicht öffnen.
Schulen: Präsenzunterricht ist nur dann möglich, wenn alle Schülerinnen und Schüler mindestens zwei Mal pro Woche getestet werden können. Ab einer Inzidenz von 100 ist Wechselunterricht vorgeschrieben - allerdings nur bis zu einer Sieben-Tage-Inzidenz von 165. Sollte der Wert darüber klettern, müssen Schulen schließen und auf Distanzunterricht wechseln.
Arbeit: Im Infektionsschutzgesetz ist die Pflicht zum Homeoffice verankert. Arbeitgeber müssen dies anbieten, "wenn keine zwingenden betriebsbedingten Gründe entgegenstehen". Ist kein Homeoffice möglich, müssen Arbeitgeber Tests anbieten.
Sport: Für Kinder im Alter bis 14 Jahren soll Sport in Gruppen weiter möglich sein.
Die Koalition einigte sich in den gemeinsamen Gesprächen außerdem darauf, dass die Bundesregierung keine Verordnungen zur Eindämmung der Pandemie am Bundestag vorbei erlassen kann.

Wo die Nordländer ihre eigenen Corona-Regeln haben
Ungeachtet der geplanten Änderungen im Infektionsschutzgesetz gelten in den norddeutschen Bundesländern derzeit in einigen Bereichen unterschiedliche Corona-Regelungen. Gegebenenfalls würden diese im Einzelfall vom Bundesgesetz außer Kraft gesetzt. Einige der wichtigsten Bestimmungen im Überblick:

Niedersachsen: Ausgangssperren und private Kontakte je nach Inzidenz
Nächtliche Ausgangssperren: Die aktuelle Corona-Verordnung des Landes vom 27. März sieht nächtliche Ausgangssperren für Kommunen mit besonders hohen Inzidenzwerten (ab 150) vor. Im Einzelfall entscheiden die Landkreise und Städte. Seit Inkrafttreten der Verordnung hatten mehrere Kommunen in Niedersachsen Ausgangsbeschränkungen angeordnet, später aber teilweise wieder aufgehoben. Hintergrund ist eine Entscheidung des Oberverwaltungsgerichts Lüneburg, das die entsprechende Regelung für die Region Hannover für rechtswidrig erklärt hatte.
Modellprojekte: In landesweit 13 ausgewählten Kommunen sollen noch im April Modellprojekte mit bestimmten Lockerungen starten. Wie diese Pläne mit den geplanten Änderungen des Infektionsschutzgesetzes vereinbar sind, ist noch unklar.
Im Einzelhandel sind Terminvereinbarungen mit Kunden möglich, sofern die Sieben-Tage-Inzidenz im jeweiligen Landkreis oder einer kreisfreien Stadt unter 100 liegt. Auf je 40 Quadratmeter Verkaufsfläche darf sich maximal ein Kunde oder eine Kundin samt Begleitperson aufhalten.
Schulen: Liegt eine Region drei Tage in Folge unter der Inzidenz von 100, findet weiter Wechselunterricht statt. Liegt sie darüber, geht es - mit Ausnahme der Grund- und Förderschulen sowie Abschlussklassen - zurück in den Distanzunterricht. Schüler müssen morgens vor dem Präsenzunterricht zu Hause einen Corona-Selbsttest machen.
Rechte für Geimpfte: Seit dem 19. April sind Menschen mit vollständigem Impfschutz in Niedersachsen von Test- und Quarantäne-Pflichten befreit. Diese Regel greift zwei Wochen nach der Zweitimpfung. Das heißt etwa, dass für Mitarbeiterinnen und Mitarbeiter in Alten- und Pflegeheimen nach der zweiten Impfung die Testpflicht entfällt.

Wie viele Menschen dürfen sich bei welcher Inzidenz treffen? Was gilt in Hochinzidenzkommunen? Die Regeln im Überblick.
Schleswig-Holstein: Außengastronomie darf wieder öffnen
In Schleswig-Holstein gilt seit dem 12. April und vorerst bis zum 9. Mai 2021 eine aktualisierte Corona-Bekämpfungsverordnung.

Außengastronomie: Seit dem 12. April darf die Außengastronomie in Regionen, die stabil unter eine Inzidenz von 100 liegen, unter strikten Auflagen öffnen. Maximal fünf Personen aus zwei Haushalten an einem Tisch, Kinder unter 14 werden nicht mitgezählt. Die Landesregierung rät dazu, vor dem Besuch der Außengastronomie Schnell- oder Selbsttests zu nutzen. Pflicht sind diese aber nicht. Alkoholische Getränke dürfen bis 21 Uhr ausgeschenkt werden.
Modellprojekte: Diese können die Gesundheitsämter für Tourismus, Sport und Kultur zeitlich befristet und räumlich abgegrenzt zulassen. Damit verbunden sind strenge Schutzmaßnahmen und Testkonzepte. Im Tourismus sind bereits vier Modellregionen ausgewählt. Start ist der 19. April beziehungsweise der 1. Mai.

Neue Corona-Regeln in SH: Das ist erlaubt, das ist verboten
Was Schleswig-Holsteiner beachten müssen - von A wie Abstandsgebot bis Z wie Zweitwohnung.
Mecklenburg-Vorpommern: Verschärfter Lockdown seit dem 19. April
Auf dem MV-Gipfel am 16. April wurde ein verschärfter landesweiter Lockdown vereinbart, der Landtag beschloss ihn am Tag darauf. Diese Maßnahmen gelten seit dem 19. April voraussichtlich bis Mitte Mai - abhängig vom Infektionsgeschehen:

Verschärfte Kontaktbeschränkungen: Ein Haushalt darf sich nur noch mit einer weiteren Person treffen, Kinder unter 14 Jahren werden nicht mitgerechnet.
Notbetrieb in Schulen und Kitas: In Kitas wird nur noch eine Notbetreuung angeboten. In Schulen dürfen nur noch die Abschlussklassen in den Unterricht, für alle anderen gilt: Unterricht zu Hause. Für die Klassenstufen 1 bis 6 wird wieder ein Notbetreuung nach Anmeldung eingerichtet. Bereits seit dem 12. April gelten in Mecklenburg-Vorpommern erweiterte Corona-Testregelungen für Kitas und Schulen. Schon ein Schnupfen reicht, damit die Eltern mit dem Kind zum Haus- oder Kinderarzt müssen, um dort einen PCR-Test vornehmen zu lassen.
Ausgangsbeschränkungen: Diese sollen von den Landkreisen und kreisfreien Städten verfügt werden. Die eigene Unterkunft beziehungsweise das eigene Grundstück darf zwischen 21 und 6 Uhr nur noch aus triftigen Gründen verlassen werden. Triftige Gründe sind zum Beispiel die Berufsausübung oder die Pflege hilfsbedürftiger Personen.
Schließungen im Einzelhandel, Dienstleistungen und Kultureinrichtungen: Körpernahe Dienstleistungen wie Massagesalons, Tattoostudios und Kosmetiksalons müssen schließen, Friseure dürfen allerdings offen bleiben. Ebenfalls geöffnet bleiben dürfen Lebensmittelmärkte, Tankstellen, Banken, Poststellen, Apotheken, Drogerien, Buchhandlungen, Baumärkte, Blumenläden und Gartenmärkte. Alle anderen Einzelhandelsgeschäfte müssen schließen. Auch Museen und Kultureinrichtungen bleiben zu. Fahrschulen dürfen nur noch Fahrschüler unterrichten, die ihren Führerschein beruflich benötigen.
Einschränkungen beim Sport: Kinder- und Jugendsport wird untersagt. Individualsport ist nur noch alleine oder zu zweit möglich.
Kein Tourismus: Wer außerhalb von Mecklenburg-Vorpommern lebt, darf während des landesweiten Shutdowns seinen Zweitwohnsitz in MV nicht aufsuchen. Das gilt auch für Dauercamper und Tagestouristen von außerhalb.

Hamburg setzt nur um, was die bisherigen Regeln verschärft
Von der "Bundes-Notbremse" wird in Hamburg nur umgesetzt, was die bisherigen Regeln noch verschärft. Möglichkeiten für Lockerungen will der Senat nicht nutzen.

Joggen und Spaziergänge nur noch bis Mitternacht: Die Ausgangsbeschränkung gilt in der Hansestadt Stadt weiterhin ab 21 Uhr - und nicht ab 22 Uhr wie im Bundesgesetz. Gleichzeitig gilt dann aber das bundesweite Verbot für Jogging-Runden und Spaziergänge ab Mitternacht.
FFP2-Masken-Pflicht in Bussen und Bahn: Künftig reicht eine einfache OP-Maske nicht mehr aus - es muss eine FFP2-Maske sein.
Kein "Click&Meet": Ein "Click&Meet" im Einzelhandel wird es nach Worten des Ersten Bürgermeisters, Peter Tschentscher auch in den kommenden Wochen nicht geben. Man bleibe in Hamburg bei "Click&Collect" und auch die Schnelltest-Strategie und die geltende Maskenpflicht im öffentlichen Raum würden fortgesetzt.
Zoo darf öffnen: Der Tierpark Hagenbeck darf wieder große und kleine Besucherinnen und Besucher empfangen. Man muss sich aber vorher testen lassen. Die Hagenbeck-Innenbereiche wie das Tropen-Aquarium bleiben allerdings dicht.
Lockerungen in Pflegeheimen: Unabhängig vom Bundesgesetz sind in der neuen Corona-Verordnung aber Lockerungen für Senioren- und Pflegeheime vorgesehen. Vollständig geimpfte Bewohner und Bewohnerinnen dürfen sich dann wieder ohne Maske und Abstand treffen. Besucherinnen und Besucher müssen sich weiterhin testen lassen, dürfen aber wie vor der Pandemie täglich kommen. Grund ist, dass es kaum noch Corona-Fälle in den Einrichtungen gibt, weil sehr viele dort geimpft sind.

Der Lockdown zur Bekämpfung der Corona-Pandemie ist grundsätzlich bis zum 18. April verlängert worden. Gleichwohl gelten nach wie vor die Beschlüsse von Anfang März weiter, die auch Öffnungsmöglichkeiten je nach regionaler Infektionslage vorsehen.

Stufenweise Öffnungsstrategie mit eingebauter "Notbremse"

Vereinbart wurde am 3. März eine Öffnungsstrategie in fünf Schritten - aber auch mit einer eingebauten "Notbremse": Führen einzelne Lockerungen zu einem starken Anstieg der Infektionszahlen in einer Region, werden alle schon erfolgten Erleichterungen wieder gestrichen. Seit dem 8. März sind wieder private Zusammenkünfte des eigenen Haushalts mit einem weiteren Haushalt möglich, jedoch beschränkt auf maximal fünf Personen plus Kinder bis 14 Jahre. Wenn die Sieben-Tage-Inzidenz an drei aufeinanderfolgenden Tagen über 100 liegt, greift eine "Notbremse". Dann gelten ab dem zweiten darauffolgenden Werktag wieder die Regeln, die bis zum 7. März griffen. Das heißt in diesem Fall: Ein Haushalt und eine weitere Person dürfen sich treffen. In Regionen mit einer Sieben-Tage-Inzidenz von unter 35 können der eigene Hausstand mit zwei weiteren Haushalten zusammenkommen - allerdings mit maximal zehn Personen.

Schrittweise Lockerungen: Bund-Länder-Übersicht

Bund und Länder haben den Lockdown verlängert. Lockerungen der Maßnahmen sollen schrittweise erfolgen.

Zweiter Schritt: Buchhandlungen und Blumenhändler dürfen öffnen

Nach den schon vorgenommenen ersten Öffnungen bei Schulen und Friseuren (erster Öffnungsschritt) folgten am 8. März in einem zweiten Schritt Buchhandlungen, Blumengeschäfte und Gartenmärkte. In einzelnen Ländern waren diese bereits offen, inzwischen werden sie nach einem Beschluss der Bund-Länder-Runde bundesweit einheitlich dem Einzelhandel des täglichen Bedarfs zugerechnet. Voraussetzung ist, dass Hygiene-Konzepte und eine Kundenbegrenzung eingehalten werden. Auch Fahr- und Flugschulen konnten den Betrieb unter Auflagen wieder aufnehmen. Sogenannte körpernahe Dienstleistungen wie etwa Massagen sind auch wieder erlaubt.

Dritter Schritt: Perspektiven für Handel und Museen

Weitere eingeschränkte Öffnungen gibt es im dritten Öffnungsschritt in Regionen, in denen eine Sieben-Tage-Inzidenz von 100
Neuinfektionen je 100.000 Einwohner unterschritten wird. Neben Terminshopping-Angeboten im Einzelhandel können dort Museen, Galerien, Zoos, botanische Gärten und Gedenkstätten für Besucher mit Terminbuchung öffnen. Erlaubt ist in diesen Regionen auch Individualsport alleine oder zu zweit sowie Sport in Gruppen von bis zu zehn Kindern bis 14 Jahren im Außenbereich. Bei einer stabilen Sieben-Tage-Inzidenz von unter 50 fallen die Auflagen weg oder werden abgeschwächt. Dann ist auch kontaktfreier Sport in kleinen Gruppen im Freien wieder möglich.

Vierter Schritt: Öffnung der Außengastronomie mit Terminbuchung

Der vierte Öffnungsschritt kann - wiederum in Abhängigkeit vom Infektionsgeschehen - erfolgen, wenn die Sieben-Tage-Inzidenz nach dem dritten Schritt in dem Land oder der Region 14 Tage lang unter 50 bleibt: Seit dem 22. März geht es dann zunächst um die Öffnung der Außengastronomie mit Terminbuchung. Ein tagesaktueller Covid-19-Test ist nötig, wenn sich mehrere Haushalte einen Tisch teilen - auch für den Besuch von Theatern, Konzert- und Opernhäusern sowie Kinos. Auch kontaktloser Hallensport sowie Kontaktsport im Außenbereich soll dann wieder erlaubt sein.

Fünfter Schritt: Einzelhandel kann wieder öffnen

Bleibt die Sieben-Tage-Inzidenz auch nach den Lockerungen vom vierten Schritt zwei Wochen lang stabil, könnten laut Stufenplan weitere Öffnungen kommen - so zum Beispiel Freizeitveranstaltungen mit bis zu 50 Menschen im Außenbereich und Kontaktsport in Hallen. Auch der Einzelhandel könnte dann profitieren: Öffnungen sind möglich, wenn die Inzidenzwerte stabil zwischen 35 und 100 bleiben. Zudem müssen Auflagen eingehalten werden - zum Beispiel darf pro zehn Quadratmeter nur ein Kunde empfangen werden.

Bundesweit weiter in Kraft sind unter anderem folgende allgemeine und oftmals bereits seit Langem gültige Regeln zur Eindämmung des Corona-Infektionsgeschehens:

Die Abstands- und Hygiene-Regeln gelten überall weiter: Auf die Einhaltung eines Mindestabstands von 1,5 Metern ist konsequent zu achten.
Masken: Das Tragen von OP-Masken oder Masken mit FFP2- oder einem vergleichbaren Standard in Geschäften und öffentlichen Verkehrsmitteln bleibt Pflicht.
Kultur: Kinos, Konzerthäuser, Musikclubs und Theater bleiben weiter geschlossen.
Hotel und Gastronomie: Noch keine Lockerungen in Sicht. Bund und Länder wollen eine "sichere und gerechte Öffnungsstrategie" entwickeln.
Sport: Profisportler können weiter unter Ausschluss von Zuschauern und mit strengen Hygiene-Konzepten Spiele und Wettbewerbe ausrichten.
Impfungen: Geplant ist weiter, dass alle Bürgerinnen und Bürger bis zum Ende des Sommers ein "Impfangebot" bekommen, also mindestens eine erste Impfung gegen Covid-19 erhalten haben. Kita-Beschäftigten sowie Lehrerinnen und Lehrern an Grundschulen wurde beim Impfen eine höhere Priorität eingeräumt.
Regeln für Gottesdienste: Geistliche Feiern, beispielsweise in Kirchen, Synagogen und Moscheen, bleiben grundsätzlich zulässig. Allerdings müssen Besucher den Mindestabstand von 1,5 Metern einhalten und auch an ihrem Platz eine medizinische Maske tragen. Gemeindegesang ist untersagt. Über die Osterfeiertage soll es nach Möglichkeit aber keine Präsenzgottesdienste geben. Religiöse Versammlungen sollen nach dem Wunsch von Bund und Ländern vom 22. März nur virtuell abgehalten werden.
Schutzvorkehrungen in Pflege- und Altenheimen: Beim Kontakt mit Bewohnern gibt es für das Personal eine Pflicht zum Tragen stärker schützender FFP2-Masken. Außerdem sollen Pflegekräfte und Besucher konsequent getestet werden.
Der Verzehr von alkoholischen Getränken im öffentlichen Raum ist in der Regel untersagt. Verstöße werden mit einem Bußgeld belegt. Die Lieferung von Speisen durch Gastronomiebetriebe und das dortige Abholen von Gerichten sind aber weiter möglich.
Homeoffice: Arbeitgeber und Arbeitgeberinnen sollen Beschäftigten das Arbeiten im Homeoffice ermöglichen, "sofern die Tätigkeiten es zulassen". So sollen Kontakte am Arbeitsort, aber auch auf dem Weg zur Arbeit reduziert werden. Wo Homeoffice nicht möglich ist, sollen immer dann, wenn sich mehrere Personen in einem Raum aufhalten, medizinische Masken getragen werden. Für einen umfassenden Infektionsschutz sollen die Mitarbeitenden einmal pro Woche das Angebot mindestens eines kostenlosen Schnelltests erhalten.
Betriebskantinen sollen nach Möglichkeit schließen und allenfalls noch Speisen und Getränke zum Mitnehmen anbieten dürfen.
Quarantäne: Die Quarantänezeit von Kontaktpersonen von mit dem Coronavirus infizierten Menschen ist seit dem 1. Dezember von 14 auf zehn Tage verkürzt. Voraussetzung ist ein negativer Corona-Test.
Reisen: Nicht notwendige private Reisen und Besuche - auch von Verwandten - sollen unterlassen werden. Das gilt auch im Inland und für überregionale tagestouristische Ausflüge.
Testpflicht bei Einreise aus Risikogebieten: Bei Einreisen aus ausländischen Risikogebieten ist die Eintragung in die digitale Einreiseanmeldung Pflicht. Neben der bestehenden zehntägigen Quarantänepflicht muss jemand, der aus einem Risikogebiet in die Bundesrepublik Deutschland einreist, spätestens 48 Stunden nach Einreise nachweisen können, dass er nicht mit dem Coronavirus infiziert ist. Einreisende aus besonders betroffenen Regionen müssen schon vor der Einreise ein negatives Testergebnis vorlegen.
Beschränkung auf 15-Kilometer-Bewegungsradius möglich: In Landkreisen mit hohen Corona-Infektionszahlen kann der Bewegungsradius auf 15 Kilometer um den Wohnort beschränkt werden. Dies gilt für Landkreise mit einer Sieben-Tage-Inzidenz von über 200 Neuinfektionen pro 100.000 Einwohner.

Vor Jahren verstarb eine Patientin mit multipler Sklerose im Alter von 12 Jahren. Am Ende erstickte sie qualvoll & voller Todesangst. Frau Schlothauer, dies zu verhöhnen ist zutiefst inhuman und widerlich. Ich bin fassungslos vor Trauer & Wut.

Dazu möchte ich anmerken:

1. Es sind nicht nur schwerstkranke, es sind auch sterbende Patienten.

2. Diese Unmenschlichkeit ist nichts Neues. Anthroposophen geben Kindern, die Keuchhusten (eine durchaus tödliche Krankheit!) haben, absichtlich "Musikinstrumente", die das grauenhafte Atemgeräusch imitieren.

Die Stadt Schwerin hat nach drei Corona-Infektionsfällen beim Praxisteam eines Chirurgen in der Weststadt die Polizei eingeschaltet. Hinweisen zufolge habe das Team trotz Corona-Symptomen weitergearbeitet. Die Patienten sollen jetzt in Quarantäne.

Am Sonntagmittag hat die Stadtverwaltung die Daten aller Patienten erhalten. Demnach könnten sich 237 Menschen in einer chirurgischen Praxis im Ärztehaus in der Schweriner Weststadt mit dem Corona-Virus infiziert haben, davon kommen 175 aus Schwerin und der Rest aus den umliegenden Landkreisen. Das bestätigte der Sozialdezernent der Stadt, Andreas Ruhl (SPD), NDR 1 Radio MV. Alle 237 Patienten müssen in Quarantäne und sich testen lassen. Das Gesundheitsamt der Stadt will alle Patienten kontaktieren, die sich zwischen dem 21. und dem 28. April länger als zehn Minuten in der Praxis in der Johannes-Brahms-Straße 59 aufgehalten haben. Sie wurden von der Stadt aufgefordert, sich sofort vorsorglich in Quarantäne zu begeben. Die Stadt hat die Quarantäne perAllgemeinverfügung förmlich angeordnet und diese als Bekanntmachung auf ihrer Internet-Seite veröffentlicht. Auch die Betroffenen in den Nachbarlandkreisen würden benachrichtigt. Etwa 150 Betroffene haben sich bereits in der Helios Klinik testen lassen. Die Ergebnisse werden frühestens am Montag erwartet."

Die drücken auf die Tränendrüse wegen der Belämmertheit der Kinder! Ich faß es nicht, ich faß es nicht.

Hätten die Großeltern und Urgroßeltern gewußt, was für ein jämmerliches Heulsusenpack sie in die Welt setzen und dafür all die Strapazen auf sich nehmen, die sie damals auf sich genommen haben, die wären auf der Stelle enthaltsam geworden und hätten nie wieder im Leben Sex gehabt.

Corona-Impfstrategie für Kinder und Jugendliche entwickeln
Berlin (ots)

Berlin, 05.05.2021 - Bundesärztekammer-Präsident Dr. Klaus Reinhardt hat vor negativen Kollateraleffekten der Corona-Eindämmungsmaßnahmen für Kinder und Jugendliche gewarnt. "Es geht dabei nicht nur um entstandene schulische Bildungsdefizite, sondern mehr noch darum, dass viele Kinder wichtige Entwicklungsphasen in sozialer Isolation erlebt haben", sagte Reinhardt auf dem 124. Deutschen Ärztetag, der in diesem Jahr coronabedingt als reine Online-Veranstaltung abgehalten wird.

In ihrer Generalaussprache auf dem Ärztetag haben die Abgeordneten eine medizinisch-wissenschaftliche Evaluation aller Kollateraleffekte von Eindämmungs- und Schutzmaßnahmen im Hinblick auf den Zugang zur medizinischen Akutversorgung und zu notwendigen Vorsorgeleistungen sowie mögliche psycho-soziale Auswirkungen des Lockdowns gefordert. Für die weitere Krisenbewältigung und zur Vorbereitung auf zukünftige pandemische Lagen sei die Entwicklung von Handlungsstrategien zur Vermeidung derartiger Kollateraleffekte unerlässlich.

In einem weiteren Beschluss hat der Ärztetag die Bundesregierung aufgefordert, unverzüglich eine Covid-19-Impfstrategie für Kinder und Jugendliche zu entwickeln. Das Recht auf Bildung könne im Winter 2021/2022 nur mit einer rechtzeitigen Corona-Impfung gesichert werden. Ohne rechtzeitige Impfung, insbesondere auch für jüngere Kinder, führe ein erneuter Lockdown für diese Altersgruppe zu weiteren gravierenden negativen Folgen für die psychische Entwicklung.

Um diese zu verhindern, sollten die Forschung zu Impfstoffen für diese Altersgruppe gezielt gefördert und die ausreichende Versorgung mit adäquaten Impfstoffen sichergestellt werden. Eine zentrale Rolle in der Impfstrategie spielen nach Auffassung des Ärzteparlaments neben den Hausärzten die Kinder- und Jugendärzte in Praxis, Klinik und Gesundheitsämtern. Sie benötigten Unterstützung bei der kurzfristigen Impfdurchführung.

Die Impfbereitschaft in UK ist sehr beeindruckend. Bei den > 50 Jährigen wird Herdenimmunität erreicht. Grosse Wellen an Sterbefällen sind so praktisch ausgeschlossen, auch mit indischer Variante. Hoffentlich erreichen wir eine ähnlich gute Impfquote. Dafür müssen wir werben.

Besonders tragisch: Auch bei Kindern ist #Longcovid nicht ungewöhnlich. Jedes 13. ist auch nach 3 Monaten noch nicht gesund. Meistens gibt es danach nur noch bei wenigen eine deutliche Verbesserung.

Abstract
Long-term complications after coronavirus disease 2019 (COVID-19) are common in hospitalized patients, but the spectrum of symptoms in milder cases needs further investigation. We conducted a long-term follow-up in a prospective cohort study of 312 patients—247 home-isolated and 65 hospitalized—comprising 82% of total cases in Bergen during the first pandemic wave in Norway. At 6 months, 61% (189/312) of all patients had persistent symptoms, which were independently associated with severity of initial illness, increased convalescent antibody titers and pre-existing chronic lung disease. We found that 52% (32/61) of home-isolated young adults, aged 16–30 years, had symptoms at 6 months, including loss of taste and/or smell (28%, 17/61), fatigue (21%, 13/61), dyspnea (13%, 8/61), impaired concentration (13%, 8/61) and memory problems (11%, 7/61). Main
The respiratory tract is the site of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) entry and infection; however, COVID-19 is a complex systemic disease, affecting the cardiovascular, renal, hematologic, gastrointestinal and central nervous systems1. As evidence emerges of predominantly lasting impairment of lung function related to fibrosis, more data on the long-term effects of COVID-19 on other organs are required2. A plethora of symptoms persist in patients surviving severe COVID-19 (refs. 3,4), and a long COVID syndrome has been proposed5,6. However, the severity and duration of symptoms remain largely unknown. Chronic fatigue occurred after SARS infection in 2003 (ref. 7), and it is well known in the aftermath of a spectrum of infectious diseases8,9,10,11,12,13. Before the SARS-CoV-2 pandemic, patient management in intensive care was frequently associated with mental and physical decline, and this could partially explain long COVID in patients with severe illness14. However, the burden of long COVID in mild to moderately ill patients is not well defined. We assessed persistent symptoms 6 months after initial COVID-19 in a prospective cohort of hospitalized and home-isolated patients from the first pandemic wave in Bergen, Norway.

Results
All patients diagnosed at the only centralized testing facility in the city of Bergen were invited to participate, as well as all patients admitted to the city’s two hospitals: Haukeland University Hospital and Haraldsplass Deaconess Hospital. Recruitment commenced with the first diagnosed home-isolated and the first hospitalized patients; 92% of patients during the first pandemic wave agreed to participate. The objective was to identify factors and biomarkers associated with long-term complications. From 28 February to 4 April 2020, we consecutively recruited 357 patients who were positive for SARS-CoV-2. We collected demographic and clinical data as well as blood samples. Household members of patients who tested positive were included to ensure completeness of the cohort, and their infection was diagnosed by SARS-CoV-2-specific antibodies at 2 months15. At 6-month follow-up, the study population available for analysis comprised 312 patients, of whom 247 were home-isolated and 65 were hospitalized (Fig. 1).

The median age of the study population was 46 years (interquartile range (IQR) 30–58 years) with 51% (160/312) women. Forty-four percent had comorbidities (137/312), the most frequent being chronic lung disease (12%, 38/312, 34 with asthma), hypertension (11%, 35/312), chronic heart disease (7%, 22/312), rheumatic diseases (6%, 20/312), diabetes (4%, 13/312) and immunosuppressive conditions (4%, 11/312). In 272 patients who recorded symptoms during acute disease, fatigue (90%, 244/272), cough (71%), headache (64%), myalgia (58%) and dyspnea (55%) were the most frequent, whereas only 21% had fever. Hospitalized patients were older than home-isolated patients, had higher body mass index (BMI) and had more comorbidities, including chronic lung disease, chronic heart disease, hypertension and diabetes (Table 1).

Sixty-one percent (189/312) of the total patient population had persistent symptoms 6 months after initial COVID-19 illness, with the most common symptoms being fatigue (37%), difficulty concentrating (26%), disturbed smell and/or taste (25%), memory problems (24%) and dyspnea (21%). Whereas the frequency of most symptoms increased with age in the study population, disturbed smell and/or taste was more frequent in people younger than 46 years old (Table 2). Thirty-nine percent of the study population, commonly children and young adults, had no symptoms at 6 months. Even among the 247 home-isolated patients, 55% (136/247) experienced persistent symptoms at 6 months, most commonly fatigue (30%), disturbed taste and/or smell (27%), concentration impairment (19%), memory loss (18%) and dyspnea (15%) (Table 2).

The youngest age group (0–15 years) rarely suffered persistent symptoms (13%, 2/16), whereas 52% (32/61) of young adults aged 16–30 years who were home-isolated for mild to moderate initial illness had persistent symptoms, the most common being disturbed taste and/or smell (28%), fatigue (21%), dyspnea (13%) and impaired concentration (13%) and memory (11%) (Table 2). In these young adults, comorbidity was not significantly associated with persistent symptoms (33% versus 31%, P = 1) or fatigue (47% versus 27%, P = 0.2), although numbers of subjects were low.

Convalescent antibodies reach a plateau approximately 1–2 months after infection, providing a general marker for the magnitude of the immune response16. SARS-CoV-2 spike protein specific IgG and microneutralizing antibody titers detected after 2 months were significantly higher in hospitalized patients than home-isolated patients (P < 0.001; Extended Data Fig. 1). Increased antibody titers at 2 months were associated with the severity of initial illness, older age and higher BMI in multivariable analysis (Table 3 and Fig. 2; severity score adapted from Beigel et al.17).

Increased antibody titers as well as pre-existing lung disease were independently associated with both persistent fatigue and total number of symptoms at 6 months in multivariable analysis (Table 4 and Fig. 2c,d,g,h). Severity of initial illness was associated with persistent fatigue and weakly associated with total number of symptoms (Table 4).

Post-viral fatigue has been reported afer SARS infection7 and other viral infections11,12,13. To assess fatigue, we used the Chalder fatigue score, which is validated for adults18,19. Fatigue is defined as a total bimodal score of 4 or higher on 11 questions. Thirty percent (69/231) of home-isolated patients, 16 years of age or older, had fatigue at 6 months compared to 63% (39/62) of hospitalized patients. Severe fatigue at 6 months, defined as bimodal score ≥4 + total ordinal score ≥23, was present in 7% (17/231) of home-isolated and 24% (15/62) of hospitalized patients (Supplementary Table 1). In home-isolated patients, the most frequent symptoms of physical fatigue (questions 1–7) were tiredness (35%, 81/231), increased need for rest (30%) and lack of energy (29%); the most common symptoms of mental fatigue (questions 8–11) were difficulties finding words (23%), difficulties concentrating (19%) and memory problems (18%) (Supplementary Table 1).

In patients 16 years of age or older, fever during acute illness, severity of initial illness and female gender were associated with fatigue at 6 months in binomial logistic regression (Supplementary Table 2). In addition to female gender, pre-existing lung disease, severity of acute illness and increased convalescent antibody titers were independently associated with increasing fatigue score at 6 months in multivariable analysis (Table 4 and Fig. 2d,h).

Despite the correlation between severity of initial disease and antibody titers at 2 months, both factors were independently associated with features of long COVID in multivariable analysis (Table 4 and Supplementary Table 2). In stratified analysis of 242 home-isolated patients with low to moderate symptoms, increased antibody titers remained associated with number of symptoms (odds ratio (OR) = 1.56, confidence interval (CI) 1.23–1.96) and fatigue score (OR = 1.07, CI 1.02–1.12), although the association was not statistically significant for fatigue as a dichotomous variable (OR = 1.48, CI 0.98–2.31).

Discussion
Our study is novel in assessing long COVID symptoms, not only in hospitalized patients but also in young patients and home-isolated patients with milder disease. A strength is a near-complete, geographically defined cohort of both antibody- and reverse-transcriptase polymerase chain reaction (RT-PCR)-positive patients from the first pandemic wave, including all severities of disease, in an immunologically naive population. The small subgroups are a limitation of this study, and our findings should be confirmed in larger cohorts.

We found that a large proportion of survivors of COVID-19 in our cohort had persistent symptoms 6 months after their initial illness. Although it has previously been reported that patients hospitalized for severe COVID-19 frequently suffer long-term symptoms20,21,22,23, we found that more than half of home-isolated, mildly to moderately ill patients with COVID-19 still suffered symptoms 6 months after infection. It is worrying that non-hospitalized, young people (16–30 years old) suffer potentially severe symptoms, such as concentration and memory problems, dyspnea and fatigue, half a year after infection. Particularly for students, such symptoms might interfere with their learning and study progress.

The high prevalence of persistent fatigue in patients with COVID-19 is striking and appears higher than observed after common infections, such as influenza, Epstein–Barr virus mononucleosis and dengue11,12,13. Data from Norway have previously shown slightly lower chronic fatigue prevalence (11%) in the general population24 than in the present household controls (14%), who were younger and had fewer comorbidities than infected patients but were sampled at the same time. However, this apparent difference might be a coincidence owing to low numbers. Our finding that women had higher prevalence of fatigue concurs with results from an earlier study in the general Norwegian population24. The association between severity of illness and persistent symptoms agrees with data from hospitalized patients with COVID-19 (ref. 20). As the respiratory tract is the main target organ for COVID-19, our finding of an association between underlying chronic lung disease (mostly asthma) and persistent symptoms, including fatigue, is not surprising.

The association between severe initial disease and increased antibody titers at 2 months could be due to higher viral load, which could trigger the immune system more profoundly25. The finding of increased convalescent antibody titers with increasing age could be explained by more severe disease in older people, as age is a known strong risk factor for severe COVID-19. However, by contrast with the immunosenescence observed after influenza infection in the elderly, multivariable analysis indicated that age and severity of illness were independently associated with increased antibody titers. These findings call for enhanced surveillance of COVID-19 mass vaccination programs. Home-isolated patients aged 16–30 years with mild COVID-19 are at risk of long-lasting dyspnea and cognitive symptoms. Considering the millions of young people infected during the ongoing pandemic, our findings are a strong impetus for comprehensive infection control and population-wide mass vaccination.

Methods
Ethical considerations
All participants, or their guardians for children younger than 16 years old, provided written informed consent. The study was approved by the Regional Ethics Committee of Western Norway (no. 118664). No compensation was provided to participants beyond reimbursing travel costs to the clinic.

Study population
As part of a prospective cohort study with long-term follow-up, we consecutively enrolled home-isolated patients diagnosed with COVID-19 in the period 28 February to 4 April 2020, during the first wave of the pandemic in Bergen, Norway. For hospitalized patients, we accepted longer inclusion due to delay in hospitalization, up to 6 May. The study population included SARS-CoV-2 RT–PCR-positive patients diagnosed at Bergen Municipality Emergency Clinic and those admitted to the two neighboring city hospitals: Haukeland University Hospital and Haraldsplass Deaconess Hospital. Household contacts of identified patients were invited to participate in the study as secondary cases (seroconverters) or seronegative controls15. All registered patients with SARS-CoV-2 were invited to participate. The initial rate of participation at inclusion was 92% (278/302); at 2 months, the rate of participation was 88% (336/381, numerator and denominator includes seropositive household members at this time point); and at 6 months, the rate of participation was 82% (312/381). Convalescent serum samples were collected 2 months after infection for detection of anti-SARS-CoV-2 antibody titers.

All testing in Bergen was centralized to the Emergency Clinic and the city’s two hospitals, allowing recruitment of all consenting patients diagnosed with COVID-19 in Bergen. In total, 3,319 patients were tested by RT–PCR in the Municipality Emergency Clinic during the period. Of these, 228 tested positive by RT-PCR, and six were excluded because their address was outside of Bergen or because they were admitted to hospital and were categorized as hospitalized. All suspected cases seen at the Municipality Emergency Clinic were examined clinically and diagnosed by medical staff. Patients with moderate disease were sent home for home isolation, or, if considered severe, they were hospitalized. Most of the hospitalized patients were diagnosed upon admission. Nasopharyngeal swabs were collected for laboratory confirmation. Patients were telephoned with their results of the RT–PCR test, which was conducted at the reference microbiology laboratory in the tertiary hospital. All confirmed home-isolated patients were contacted by telephone with an invitation to join the study. An additional 79 patients were identified through investigation of seroconversion of household members of RT–PCR-positive patients. Telephone interviews were conducted by medical staff to collect clinical and demographic data. Participants attended the University Clinic at 2 months (6–8 weeks) and 6 months (±1 month) for follow-up appointments with medical staff where they were interviewed about long-term symptoms.

Clinical data
All consenting patients attended a follow-up clinic and were interviewed by medical staff at baseline, 2 months and 6 months. They provided demographic information; clinical information on symptoms at baseline and 6-month follow-up; and information on potential risk factors, including comorbidities and use of medication. Specific symptoms recorded during acute illness included fever, cough, dyspnea, fatigue, myalgia and headache. Participants were asked if they had any comorbidities and, if so, which specific comorbidities from the following: asthma, chronic obstructive lung disease, chronic heart disease, hypertension, chronic liver disease, kidney disease, neuromuscular disease, dementia, rheumatic disease, active cancer, other severe chronic disease and immunosuppressive conditions, including genetic immunodeficiency, HIV, organ transplant, and cytostatic or other immunosuppressive treatment. Data were collected on severity of initial illness—that is, need for hospitalization, symptoms during acute illness and need for non-invasive ventilatory support or respirator treatment17. Radiological investigations were conducted only on hospitalized patients. At 6-month follow-up, all participants aged 16 years or older were invited to complete a validated fatigue questionnaire containing 11 key questions according to the Chalder fatigue scale18,19. Fatigue was defined as a total dichotomized score of 4 or higher. Severe fatigue was defined as fatigue plus a total Chalder score of 23 or higher.

Laboratory methods
Diagnosis of COVID-19 was based on RT–PCR on samples from nasopharyngeal swabs and on serological evidence of SARS-CoV-2 antibody positivity26. Serum samples were collected 2 months after infection for detection of anti-SARS-CoV-2 antibody titers and stored at −80 °C until analyzed. Samples were heat-inactivated for 1 h at 56 °C before analysis in duplicate by a two-step ELISA for detecting SARS-CoV-2-specific IgG antibodies (Southern Biotech, cat. no. 2040-05) to the receptor-binding domain (RBD) (screening 1:100 dilution) and the spike protein (confirmation from 1:100 in five-fold dilutions). Endpoint titers were calculated as the reciprocal of the serum dilution giving an optical density value of 3 standard deviations above the mean of historical pre-pandemic serum samples (n = 128)26. Sera with antibodies against the RBD were tested in a microneutralization assay using the local isolate hCoV-19/Norway/Bergen-01/2020 (GISAID accession ID EPI_ISL_541970) in a certified Biosafety Level 3 laboratory as previously described27. Briefly, sera were tested in duplicate in doubling dilutions starting from 1:20 dilution and mixed with 100 TCID50 viruses, followed by incubation with rabbit monoclonal IgG against SARS-CoV2 NP (Sino Biological, cat. no. 40143-R019-100) and biotinylated goat anti-rabbit IgG (H+L) (Southern Biotech, cat. no. 4050-08) and extravidin–peroxidase (Sigma-Aldrich, cat. no. E2886). The microneutralizing antibody titer is the reciprocal of the serum dilution giving 50% inhibition of virus infectivity. For all control individuals, negative serology was confirmed at 6 months.

Statistical analysis
Data were entered using electronic case report forms in REDCap (Research Electronic Data Capture, Vanderbilt University). All analyses were conducted in R version 4.0.3 (www.r-project.org), and graphs were produced in R using the ggplot and gridExtra packages. Patients who responded to the questionnaire were included in the analysis, and results are presented as percentages with means or medians and 95% CIs. In univariable analysis, categorical variables were compared using the chi-square test and binomial logistic regression and presented with ORs, 95% CIs and P values.

Multivariable analysis was performed by binary logistic regression for dichotomous outcome variables (Supplementary Tables 1 and 2). We used negative binomial regression employing the MASS package in R to analyze factors associated with numeric outcome variables (Table 4)—that is, ‘number of symptoms’, encoded as integers from 0 to 13, according to symptoms listed in Table 2, and fatigue score according to the Chalder scale encompassing values from 0 to 33. For convalescent antibody titer as outcome variable, we log-transformed the titer values to obtain near-normal distribution and performed linear regression and reported results as unadjusted and adjusted geometric mean ratios for univariable and multivariable analysis, respectively (Table 3). In Tables 3 and 4, we included a priori potential risk factors of interest but omitted rare occurrences, and, for multivariable analysis, we included gender as well as variables that had a significance level of P < 0.1 in univariable analysis. Owing to strong collinearity between spike IgG and microneutralizing antibody titers, and because microneutralizing antibodies are a proportion of total IgG, we omitted microneutralizing antibodies from the multivariable analysis.

The analysis focuses on the potential effect of antibody titers as an exposure variable on fatigue and symptom score, respectively, as outcome variables. We assessed the confounding and effect size modification of all other exposure factors that were significant in univariable analysis.

Severity of illness was classified using an eight-category ordinal scale, as previously published17. The categories are as follows: 1—not hospitalized and no limitations of activities; 2—not hospitalized, with limitation of activities, home oxygen requirement or both; 3—hospitalized, not requiring supplemental oxygen and no longer requiring ongoing medical care (used if hospitalization was extended for infection control or other non-medical reasons); 4—hospitalized, not requiring supplemental oxygen but requiring ongoing medical care (related to COVID-19 or to other medical conditions); 5—hospitalized, requiring any supplemental oxygen; 6—hospitalized, requiring non-invasive ventilation or use of high-flow oxygen devices; 7—hospitalized, receiving invasive mechanical ventilation or extracorporeal membrane oxygenation; and 8—death.

Die Gesundheitsministerkonferenz (GMK) ermöglicht sogenannte Kreuzimpfungen mit Impfstoff von Astrazeneca (Erstimpfung) und mRNA-Impfstoffen (Zweitimpfung). Damit setzen die Gesundheitsministerinnen und -minister sowie -senatorinnen und -senatoren gemeinsam mit dem Bundesgesundheitsministerium die neue Empfehlung der Ständigen Impfkommission (STIKO) von Donnerstag um. Das haben die Mitglieder der GMK am Freitag in einer Videokonferenz einstimmig beschlossen.

Der GMK-Vorsitzende, Bayerns Gesundheitsminister Klaus Holetschek, sagte in Nürnberg: „Nach Abschluss des offiziellen Stellungnahmeverfahrens zu dem Thema bieten wir allen so bald wie möglich eine Zweitimpfung mit einem mRNA-Impfstoff an, die aktuell eine Erstimpfung mit Astrazeneca bekommen haben. Auch bei künftigen Erstimpfungen mit Astrazeneca wollen wir den Impflingen eine Zweitimpfung mit mRNA-Impfstoff anbieten."

Der GMK-Vorsitzende erklärte: „Ein klarer Vorteil ist, dass wir damit bei Kreuzimpfungen einen kürzeren Abstand von vier Wochen zwischen Erst- und Zweitimpfung haben. Einen weiteren Vorteil sehe ich darin, dass die Wirksamkeit einer Kombination aus Astrazeneca als Erstimpfung und mRNA-Impfstoff als Zweitimpfung nach neuesten wissenschaftlichen Erkenntnissen und Darstellung der STIKO mindestens so hoch ist wie eine homologe Impfserie mit einem mRNA-Impfstoff."

Der Bund beabsichtigt, den notwendigen mRNA-Impfstoff dafür bereitzustellen.

Holetschek ergänzte: „Wir haben der STIKO aber auch zu verstehen gegeben, dass kurzfristige Kommunikation zu dem in unserer Bevölkerung so wichtigen Thema des Impfens gegen das Coronavirus Irritationen auslösen kann. Ziel muss es immer sein, die Bürgerinnen und Bürger nicht zu verunsichern, sondern über Neuerungen und wichtige Erkenntnisse der Wissenschaft verlässlich und rechtzeitig zu informieren. Für künftige Fragestellungen sollten wir uns nach meiner Ansicht sicherlich nach der Pandemie auch über die gemeinsamen Strukturen austauschen."

Mir kam die Zahl ungewöhnlich klein vor. Inzwischen wurde sie nach oben berichtigt. Jetzt bei ORF2:

Inzwischen sind die Zahlen weiter geändert worden: 8000 Deppen, die nach Kroatien gefahren sind, und 300 Infizierte.

Wenn man sehr vorsichtig schätzt und bei nur 1/3 der Infizierten bleibende Schäden annimmt, werden sich 100 dieser Deppen für den Rest ihres Lebens daran erinnern können, was sie sich für ihr Radaugegeifere in Kroatien eingehandelt haben. Mangelnde Hirnleistung kann man bei allen 8000 annehmen. Wer klar denken kann, geht nicht auf solche Selbstmordtouren.

Das RKI lügt. Das RKI tut so, als ob die Kurven die Wirkung der Impfung(en) zeigen. Die Wahrheit ist jedoch, daß die Kurven die GESAMTwirkung aller Maßnahmen (und auch Fehler der Maßnahmen) zeigen. Die Impfung ist nur ein Teil der Maßnahmen. Lockdowns, Maskenpflicht, Home-Office und Schließungen von Schulen und Kindertagesstätten sind ENTSCHEIDENDE und sehr wirksame Maßnahmen.

Lest die Kommentare in dem Thread bei Twitter!

Die Wirkung der Impfungen läßt nach, weil Delta und andere Varianten den Impfschutz aushebeln. Die Impfung ist nach wie vor wichtig, aber leider bei weitem nicht so wirkungsvoll wie suggeriert wird.

Das kann nur eines heißen:

Die Maßnahmen müssen verschärft werden!
Keine Lockerungen! Sondern Impfen, Impfen, Impfen!

Vor 10 Minuten traf diese Pressemitteilung aus Bayern ein. Darin wird indirekt eingeräumt, daß die Impfstoffe von AstraZeneca und Johnson&Johnson eine deutlich geringere Wirksamkeit haben als die von Biontech und Moderna.

Die Gesundheitsministerkonferenz (GMK) hat sich für eine Fortsetzung der epidemischen Lage über den 11. September hinaus ausgesprochen. Das haben die Mitglieder der GMK am Montag einstimmig in einer Videokonferenz beschlossen.

Der GMK-Vorsitzende und bayerische Gesundheitsminister Klaus Holetschek sagte am Montagabend in München: „Wir bitten den Bundestag, die Feststellung der epidemischen Lage von nationaler Tragweite über den 11. September hinaus fortzuführen. Das ist notwendig, um Infektionsschutzmaßnahmen in den Ländern weiterhin umzusetzen, wenn dies geboten ist. Angesichts steigender Infektionszahlen halten wir das für wichtig, denn wir dürfen uns nicht voreilig unserer erprobten Instrumente berauben."

Zudem haben die GMK-Mitglieder die Zielgruppe für den Start der Corona-Auffrischungsimpfungen präzisiert. Demnach soll es zunächst Auffrischungsimpfungen geben in Pflegeeinrichtungen, Einrichtungen der Eingliederungshilfe und weiteren Einrichtungen mit vulnerablen Gruppen sowie für Menschen mit Immunschwäche oder Immunsuppression und Pflegebedürftige in ihrer eigenen Häuslichkeit sowie Höchstbetagte (ab 80 Jahren). Voraussetzung ist, dass die Zweitimpfung mindestens sechs Monate zurückliegt. Das Bundesgesundheitsministerium soll schnellstmöglich die Aufklärungsbögen für Auffrischungsimpfungen anpassen. Zudem müssen die technischen Voraussetzungen für Meldungen an das DIM-Register des Robert Koch-Instituts und die Ausstellung eines digitalen Impfzertifikats geschaffen werden.

Die GMK hatte am 2. August beschlossen, die Auffrischungsimpfungen ab September zu ermöglichen. Möglich sind auch Auffrischungen für Menschen, die mit Vektorimpfstoffen von Astrazeneca oder Johnson&Johnson geimpft wurden oder nach einer Genesung von COVID-19 einen dieser Vektorimpfstoffe erhalten haben.

Herd immunity “is not a possibility” because the delta variant can spread among vaccinated individuals, according to experts including the director of the Oxford Vaccine Group.

In a parliamentary panel on Tuesday, Professor Sir Andrew Pollard warned that herd immunity is a “mythical” concept and should not inform the design of vaccination programmes in the UK or across the globe.

“This virus is not measles – if you have 95 per cent of people vaccinated against measles, the virus cannot transmit in the population,” said Sir Andrew. “But we know very clearly with the coronavirus that the delta variant will still infect people who have been vaccinated, and that does mean that anyone who’s still unvaccinated, at some point, will meet the virus.”

He added that vaccines may slightly slow the transmission process, as there is some evidence that inoculated people are infectious for a shorter period of time, but warned “we don't have anything” which will completely halt the spread of Covid-19.

Sir Andrew’s comments were echoed by several other experts offering evidence to the All-Party Parliamentary Group (APPG) on coronavirus.

Paul Hunter, a professor of medicine at the University of East Anglia, said herd immunity is now “unachievable” while Devi Sridhar, a professor of global public health at Edinburgh University, warned it is unlikely that we will hit a “magical threshold” were the spread of Covid-19 “disappears”.


The comments come amid a shifting understanding of the role vaccinations can play to reduce the circulation of Covid-19. There had been some hopes that, if coverage was high enough, the virus would no longer be able to spread - stopping the pandemic in its tracks and conferring some protection for those who are unable to have a shot.

Earlier this year, there were even suggestions that those most likely to spread Sars-Cov-2, including frontline workers and young people, should be vaccinated first.

But over the last fortnight these hopes have unravelled. While vaccines have been shown to protect against severe illness, hospitalisation and deaths, mounting data suggests they do not halt transmission.

Mounting evidence that jabs don't halt transmission

Data published last week by Public Health England found there is little difference in how much virus is present in vaccinated and unvaccinated people who test positive for Covid-19, suggesting the shots do not suppress viral replication as much as hoped. Scientists had believed that a lower viral load would prevent onward transmission.

The study chimes with evidence from the United States, where a recent study of an outbreak in Massachusetts found viral loads were similar among 127 fully vaccinated people and 84 others who were unvaccinated.

The research led the US Centres for Disease Control and Prevention (CDC) to reverse guidance which had stated fully vaccinated people do not need to wear a face mask indoors.

But other experts have stressed that, even with similar loads, vaccinated people are unlikely to transmit at the same rate as those unvaccinated - and more research is needed to confirm the research.

“Even if viral load may be the same, vaccinated people who become infected are less likely to be infectious than unvaccinated because vaccines reduce virus shedding time, symptomatic infection, and the presence of immune response will suppress the viable virus,” said Dr Muge Cevik, a clinical lecturer in infectious diseases and virology at St Andrews University.

However, the early findings have significant implications for the future of the pandemic.

It could make travel more difficult – as vaccination status alone may not be an indication of whether an individual could transport Covid to another country – and may also change the paradigm in discussions around whether to vaccinate children.


Guidance from the UK’s Joint Committee on Vaccinations and Immunisations currently says that 16-17 year olds should be given Covid-19 jabs but, unlike a range of countries including the US and Germany, has stopped short of recommending younger children have shots.

“One of the strongest arguments that has been repeated is to vaccinate children to protect adults,” said Sir Andrew. “[But] vaccinating children is not going to completely block transmission, so it doesn’t achieve that goal.”

He added that, instead, leaders should use available vaccines to protect as many of the most vulnerable people across the globe as possible.

“Every dose makes a difference if it’s given to people whose lives might be lost this year,” he said. “We shouldn’t belittle the value of even a small donation of doses to countries where people may die from the virus.”

Francois Balloux, director of the Genetics Institute at University College London, added: “It is not so much anymore a ‘duty to others’ to get vaccinated but a protection for oneself. There won’t be any ‘herd immunity wall’ to hide behind.

“There is an important fraction of the population who for all sorts of reasons are immunocompromised. “For the vast majority of them, they still benefit from being vaccinated. Some won’t. Widespread vaccination reduces the risk of exposure to them, but only very marginally so.”

Was das RKI bei dieser Grafik verschweigt: Daß fast alle Impfungen erfolgten VOR dem Eintreffen der Delta-Variante in Deutschland. Die Varianten vor Delta konnten durch die Impfung in Schach gehalten werden. Delta hingegen durchbricht den Impfschutz.

Nearly 72,000 children tested positive for COVID-19 last week, a “substantial” increase that troubles public health advocates amid the growing wave of infections linked to the highly transmissible delta variant of the virus.

The American Academy of Pediatrics released the figures on Tuesday, noting they were a nearly 85% increase from a week earlier. Children — defined by individual states as those aged 17 or 18 and under ― now account for 19% of the nation’s weekly COVID-19 cases.

More than 4.2 million children have tested positive for the coronavirus since the pandemic began.

The data is troubling as many children are not yet eligible for a coronavirus vaccine. Those used in the United States are only authorized for people aged 12 and over, but many schools are preparing to hold in-person classes in the fall for the first time since the pandemic took hold last year.

The American Academy of Pediatrics noted that severe illness and hospitalization remained uncommon among children. Top vaccine makers are testing their products on children as young as 6 months old, but emergency use authorization is still months away.

“There is an urgent need to collect more data on longer-term impacts of the pandemic on children, including ways the virus may harm the long-term physical health of infected children, as well as its emotional and mental health effects,” the group said.

A recent study from scientists in London found that children who are infected with the coronavirus rarely develop long-term illnesses, known colloquially as “long COVID,” suggesting the issue may be more frequent in adults. Researchers found just 4.4% of kids have symptoms that last for four weeks or longer, and just 1.8% have illnesses that go on for more than eight weeks.

The report comes less than two weeks after the American Academy of Pediatrics recommended all kids over the age of 2 wear face masks in school this fall, regardless of vaccination status, citing the delta variant.

Vaccines remain the best defense against severe cases of COVID-19 and death associated with the disease, and studies show that all vaccines used in the U.S. protect well against the delta strain. Almost all of the nation’s deaths linked to the pandemic are among unvaccinated Americans, and regions with notably low inoculation rates have seen cases jump dramatically since June.

Dabei sollten sich die Ärzte und die Impfzentren - diese vor allem mit ihren mobilen Teams - zunächst auf die Pflegeeinrichtungen konzentrieren, denn dort leben die Menschen, die zuerst geimpft wurden."

August richten sich die Drittimpfungen zunächst insbesondere an Bewohnerinnen und Bewohner von Pflegeeinrichtungen sowie Einrichtungen der Eingliederungshilfe und weiteren Einrichtungen mit vulnerablen Gruppen, an Patienteninnen und Patienten mit Immunschwäche oder Immunsuppression, an Pflegebedürftige in ihrer eigenen Häuslichkeit und an Menschen ab 80 Jahren. Dabei sollen sie zunächst durch ihre behandelnden Ärztinnen und Ärzte eine Auffrischungsimpfung erhalten. Die staatlichen Angebote für Auffrischungsimpfungen sind hier ergänzend vorgesehen. Voraussetzung ist zudem, dass die Zweitimpfung mindestens sechs Monate zurückliegt. Vorgesehen ist ein mRNA-Impfstoff, unabhängig davon, mit welchem Impfstoff die erste Impfserie abgeschlossen wurde.

Holetschek erläuterte: „Wir haben am 27. Dezember 2020 mit den Impfungen begonnen. Sechs Wochen später gab es die ersten Zweitimpfungen. Folglich können inzwischen die ersten Bürgerinnen und Bürger aus der Zielgruppe, die wir definiert haben, eine Drittimpfung erhalten."

Der Minister betonte: „Selbstverständlich ist dieses Angebot zusätzlich zur Möglichkeit gedacht, dass die Heime eine Impfung durch die sie betreuenden niedergelassenen Ärzte vornehmen lassen wollen. Ich erwarte jetzt keinen Ansturm auf die Impfzentren, denn dies ist kein allgemeiner Aufruf zu Auffrischungsimpfungen. Das Angebot richtet sich zunächst nur an die Bürgerinnen und Bürger mit dem größten Risiko - und auch die Zahl derjenigen, bei denen die erste Impfserie sechs Monate zurückliegt, wird erst nach und nach steigen."

Der Minister wirbt zudem weiterhin für Erst- und Zweitimpfungen für alle, die noch nicht geimpft oder unentschlossen sind. Holetschek bekräftigte: „Die Auffrischungsimpfungen werden nicht zu Lasten der Erst- und Zweitimpfungen gehen. Wir haben genügend Impfstoff. Erstimpfungen haben weiterhin höchste Priorität! Daher ermuntere ich auch alle Pflegekräfte in den Einrichtungen, die noch nicht geimpft sind, sich unbürokratisch mitimpfen zu lassen, wenn ein mobiles Team für Auffrischungen vorbeikommt."

Bislang stehen noch keine Aufklärungsbögen für die COVID-19-Impfung zur Verfügung, die Auffrischungsimpfungen thematisieren. Solange dies der Fall ist, erfolgt die Aufklärung mündlich durch den behandelnden Arzt.

Aktuelle Daten zeigen, wie wichtig Impfungen im Kampf gegen die Corona-Pandemie sind


Aktuelle Daten aus Bayern zeigen einen klaren Trend, dass Corona-Impfungen die Inzidenzzahl deutlich absenken. Darauf hat Bayerns Gesundheitsminister Klaus Holetschek am Mittwoch hingewiesen. Der Minister betonte: „Derzeit (Stand 17.08.) liegt die 7-Tage-Inzidenz bei Ungeimpften in Bayern bei 58 pro 100.000 Einwohner. Dagegen beträgt sie bei Geimpften in Bayern nur 5,75 pro 100.000 Einwohner. Das zeigt, wie wichtig Impfungen im Kampf gegen die Corona-Pandemie sind - auch für eine Rückkehr zu möglichst vielen Freiheiten und mehr Normalität."

Holetschek fügte hinzu: „In Bayern sind mittlerweile mehr als 7,4 Millionen Menschen vollständig geimpft. Die Impfquote liegt damit bei rund 56,5 Prozent. Doch die Inzidenzen steigen wieder. Aktuell (18.08.) liegt die 7-Tage-Inzidenz in Bayern schon bei 31. Vor nur einer Woche (10.08.) lag die 7-Tage-Inzidenz noch bei 16. Wir müssen weiterhin mit Vorsicht und Umsicht agieren. Außerdem gelten weiterhin die bekannten AHA-L-Regeln, die ein möglichst hohes Schutzniveau sicherstellen sollen."

Der Minister betonte: „Die beginnende vierte Welle zeichnet sich unverkennbar ab. Klar ist auch: Die Infektionszahlen werden - auch aufgrund der besonders ansteckenden Delta-Variante - insbesondere bei ungeimpften Menschen deutlich steigen. Zeitversetzt werden dann auch aus dieser Gruppe Menschen in die Krankenhäuser eingeliefert."

Holetschek erläuterte: „Ein voller Impfschutz ist nicht nur eine verantwortungsvolle Entscheidung für die eigene Gesundheit, sondern schützt auch das Gesundheitssystem vor einer Überlastung. Die Entscheidung sich nicht impfen zu lassen, führt im Einzelfall genau zum Gegenteil. Für mich steht außer Frage, dass sich künftige Parameter stärker an der konkreten Belastung des Gesundheitssystems, also der Krankenhäuser, und an der Frage des individuellen Impfschutzes orientieren müssen."

Der Minister unterstrich: „Ich appelliere an alle, die bisher noch gezögert haben: Lassen Sie sich impfen! Gehen Sie ins Impfzentrum, zum Arzt oder nutzen Sie eine unserer zahlreichen mobilen Impf-Aktionen. Der Vergleich der Inzidenzen zeigt deutlich: Die Impfung schützt Sie und Ihre Mitmenschen. Wichtig ist mir auch dieser Hinweis: Ein Impfdurchbruch ist zwar nicht völlig auszuschließen (derzeit etwa 0,03 Prozent der rund 7,4 Millionen vollständig Geimpften in Bayern), verläuft aber in der Regel selbst bei vulnerablen Gruppen mild."

Unsere Studie umfaßt ganz Bayern. Die Zahlen liegen vor. Damit ergibt sich:

5,75/58 = 0,099137931

1 - 0,099137931 = 0,900862069

--> 90,08 Prozent.

Das heißt: Wenn Jemand infiziert wird, so ist die Wahrscheinlichkeit dafür 90 Prozent bei einem Geimpften, daß er es NICHT wird.

Ich behaupte, diese Statistik ist falsch.

Ich sage: Man muß die Gesamtmenge nehmen: 5,75 + 58 = 63,75

Dann ist die Wahrscheinlichkeit, infiziert zu werden, für einen Geimpften: 5,75/63,75 = 0,090196078

Also 9 Prozent.


Die Wahrscheinlichkeit, infiziert zu werden für einen Ungeimpften: 58/63,75 = 0,909803922

Also 91 Prozent.


Wenn der Infektionswert für beide gleich hoch ist, beispielsweise 40 zu 40, sieht das so aus:

40/80 = 0,50 --> 50 Prozent. Also Mittelwert. Also ist die Impfung wirkungslos.


Bei der Pfizer-Methode gäbe es: 40/40 = 1

1 - 1 = 0.

Die Wirksamkeit der Impfung wäre also 0.




In den nächsten Wochen wird man sehen, wie diese Zahlen sich ändern, und vor allem schnell ändern. Durch die Durchbruchsinfektionen durch die sehr aggressive Delta-Variante wird sich die Zahl für die Geimpften verschlechtern. Und zwar erheblich verschlechtern. Dagegen gibt es nur ein Mittel: IMPFEN, IMPFEN, IMPFEN !!!!!!!!! Retten, was noch zu retten ist.

Oder sich gleich erschießen.

FDA Approves First COVID-19 Vaccine
Approval Signifies Key Achievement for Public Health

Today, the U.S. Food and Drug Administration approved the first COVID-19 vaccine. The vaccine has been known as the Pfizer-BioNTech COVID-19 Vaccine, and will now be marketed as Comirnaty (koe-mir’-na-tee), for the prevention of COVID-19 disease in individuals 16 years of age and older. The vaccine also continues to be available under emergency use authorization (EUA), including for individuals 12 through 15 years of age and for the administration of a third dose in certain immunocompromised individuals.

“The FDA’s approval of this vaccine is a milestone as we continue to battle the COVID-19 pandemic. While this and other vaccines have met the FDA’s rigorous, scientific standards for emergency use authorization, as the first FDA-approved COVID-19 vaccine, the public can be very confident that this vaccine meets the high standards for safety, effectiveness, and manufacturing quality the FDA requires of an approved product,” said Acting FDA Commissioner Janet Woodcock, M.D. “While millions of people have already safely received COVID-19 vaccines, we recognize that for some, the FDA approval of a vaccine may now instill additional confidence to get vaccinated. Today’s milestone puts us one step closer to altering the course of this pandemic in the U.S.”

Since Dec. 11, 2020, the Pfizer-BioNTech COVID-19 Vaccine has been available under EUA in individuals 16 years of age and older, and the authorization was expanded to include those 12 through 15 years of age on May 10, 2021. EUAs can be used by the FDA during public health emergencies to provide access to medical products that may be effective in preventing, diagnosing, or treating a disease, provided that the FDA determines that the known and potential benefits of a product, when used to prevent, diagnose, or treat the disease, outweigh the known and potential risks of the product.

FDA-approved vaccines undergo the agency’s standard process for reviewing the quality, safety and effectiveness of medical products. For all vaccines, the FDA evaluates data and information included in the manufacturer’s submission of a biologics license application (BLA). A BLA is a comprehensive document that is submitted to the agency providing very specific requirements. For Comirnaty, the BLA builds on the extensive data and information previously submitted that supported the EUA, such as preclinical and clinical data and information, as well as details of the manufacturing process, vaccine testing results to ensure vaccine quality, and inspections of the sites where the vaccine is made. The agency conducts its own analyses of the information in the BLA to make sure the vaccine is safe and effective and meets the FDA’s standards for approval.

Comirnaty contains messenger RNA (mRNA), a kind of genetic material. The mRNA is used by the body to make a mimic of one of the proteins in the virus that causes COVID-19. The result of a person receiving this vaccine is that their immune system will ultimately react defensively to the virus that causes COVID-19. The mRNA in Comirnaty is only present in the body for a short time and is not incorporated into - nor does it alter - an individual’s genetic material. Comirnaty has the same formulation as the EUA vaccine and is administered as a series of two doses, three weeks apart.

“Our scientific and medical experts conducted an incredibly thorough and thoughtful evaluation of this vaccine. We evaluated scientific data and information included in hundreds of thousands of pages, conducted our own analyses of Comirnaty’s safety and effectiveness, and performed a detailed assessment of the manufacturing processes, including inspections of the manufacturing facilities,” said Peter Marks, M.D., Ph.D., director of FDA’s Center for Biologics Evaluation and Research. “We have not lost sight that the COVID-19 public health crisis continues in the U.S. and that the public is counting on safe and effective vaccines. The public and medical community can be confident that although we approved this vaccine expeditiously, it was fully in keeping with our existing high standards for vaccines in the U.S."

FDA Evaluation of Safety and Effectiveness Data for Approval for 16 Years of Age and Older
The first EUA, issued Dec. 11, for the Pfizer-BioNTech COVID-19 Vaccine for individuals 16 years of age and older was based on safety and effectiveness data from a randomized, controlled, blinded ongoing clinical trial of thousands of individuals.

To support the FDA’s approval decision today, the FDA reviewed updated data from the clinical trial which supported the EUA and included a longer duration of follow-up in a larger clinical trial population.

Specifically, in the FDA’s review for approval, the agency analyzed effectiveness data from approximately 20,000 vaccine and 20,000 placebo recipients ages 16 and older who did not have evidence of the COVID-19 virus infection within a week of receiving the second dose. The safety of Comirnaty was evaluated in approximately 22,000 people who received the vaccine and 22,000 people who received a placebo 16 years of age and older.

Based on results from the clinical trial, the vaccine was 91% effective in preventing COVID-19 disease.

More than half of the clinical trial participants were followed for safety outcomes for at least four months after the second dose. Overall, approximately 12,000 recipients have been followed for at least 6 months.

The most commonly reported side effects by those clinical trial participants who received Comirnaty were pain, redness and swelling at the injection site, fatigue, headache, muscle or joint pain, chills, and fever. The vaccine is effective in preventing COVID-19 and potentially serious outcomes including hospitalization and death.

Additionally, the FDA conducted a rigorous evaluation of the post-authorization safety surveillance data pertaining to myocarditis and pericarditis following administration of the Pfizer-BioNTech COVID-19 Vaccine and has determined that the data demonstrate increased risks, particularly within the seven days following the second dose. The observed risk is higher among males under 40 years of age compared to females and older males. The observed risk is highest in males 12 through 17 years of age. Available data from short-term follow-up suggest that most individuals have had resolution of symptoms. However, some individuals required intensive care support. Information is not yet available about potential long-term health outcomes. The Comirnaty Prescribing Information includes a warning about these risks.

Ongoing Safety Monitoring

The FDA and Centers for Disease Control and Prevention have monitoring systems in place to ensure that any safety concerns continue to be identified and evaluated in a timely manner. In addition, the FDA is requiring the company to conduct postmarketing studies to further assess the risks of myocarditis and pericarditis following vaccination with Comirnaty. These studies will include an evaluation of long-term outcomes among individuals who develop myocarditis following vaccination with Comirnaty. In addition, although not FDA requirements, the company has committed to additional post-marketing safety studies, including conducting a pregnancy registry study to evaluate pregnancy and infant outcomes after receipt of Comirnaty during pregnancy.

The FDA granted this application Priority Review. The approval was granted to BioNTech Manufacturing GmbH.
